The Effect of Proton Pump Inhibitor ¡¯On-demand¡¯ Therapy on GERD in Children

Abstract

|
Background/Aims: The purpose of this prospective study was to evaluate the effect of proton pump inhibitor (PPI) ¡¯on-demand¡¯ therapy on gastro-esophageal reflux disease (GERD) in children.
Methods: After 8 week-treatment with lansoprazole in patients with erosive reflux disease (ERD) or non-erosive reflux disease (NERD), 14 ERD and 10 NERD patients who showed partial improvement were treated with ¡¯on-demand¡¯ therapy for another 16 weeks. We evaluated symptom score, compliance, and adverse event at week 8 and week 16.
Results: In ERD patients, improvement was achieved in 50.0% (7/14) at week 8 and 71.4% (10/14) at week 16. In NERD patients, the rates were 60.0% (6/10) at week 8 and 80.0% (8/10) at week 16. Adverse event was found in one case (4.2%) with NERD, who had lower abdominal pain.
Conclusions: PPI was very effective and safe in treatment of children with ERD or NERD. The compliance of medication was good. In patients with partial improvement after 8 weeks¡¯ initial treatment, ¡¯on-demand¡¯ therapy was very useful as a maintenance therapy, and ¡¯on-demand¡¯ therapy for the additional 16 weeks was more effective than 8 weeks¡¯ treatment.
